The Manufacturing Site Tab requires the names of the reporting company and its manufacturing site that this declaration form is linked to.
Please note that if you have multiple manufacturing sites, you will have to complete a separate declaration form for each of the manufacturing sites.
You'll then need to provide the certification status of your manufacturing sites (if applicable) which are producing customer own-brand label products that contain coffee material. This includes the Certification Number of the certification that is relevant to the manufacturing site and the Expiry Date of the certification.
Product Information
Customer's name - Select the customer that this product is sold to.
The regional market that the customer sells this product into. Choose this from the drop-down list provided.
The commercial category that this product falls under. Select from the drop-down list the category that best represents the product supplied to this customer.
Product name - Enter the product name as used by the customer.
Unique product reference or code (UPC) used by the customer for this product.
Type of coffee product (e.g. beans grounds, power or liquid) used for the production of this product. Select from the drop-down the coffee type used in this product. If the product uses two different coffee types (e.g. beans and liquid), please input two different lines of information.
The name of the direct supplier that supplied the coffee product to this manufacturing site.
Name of the original coffee trader or importer. This is the company that purchased the coffee beans from the country of origin and brought it into the manufacturing market. This could possibly be the same name as the supplier. If you do not know this information please contact your supplier to get this information.
The name of the coffee roaster that roasted the coffee beans used for further processing and use in this product. Please include the country of processing, in the format Roaster Name - Country.
The next step is to input the country and sub-national region of origin of the coffee product used in this product. This is the area where the coffee was cultivated. If you do not know this information, please write "Unknown".
Finally, the level of traceability is needed - this is the degree to which the coffee-based material used in this product can be traced upstream in its supply chain in 2024. Please select from the drop-down list between Cooperative (i.e. the group of agricultural organisations/farms that the beans are sourced from is known), Farm/Farmer (i.e. the specific farm is known) and Farm with Geo-location (i.e. the farm can be geo-located using coordinates/polygons available).
Weight per product (UPC) and the unit of reporting (e.g. 100 grams or 0.1 kilograms). If the product is a multipack, please treat the multipack as a single product i.e. use the total weight of the multipack.
Coffee per product and the unit of reporting. Disclose the coffee per product either in terms of weight (e.g. grams or kilograms) or in terms of percentage. This is the amount of the coffee-based material present within each unit of the product.
Total tonnage of the coffee-based material used in this product is automatically calculated based on the Weight per product, Coffee per product and the 2024 UPC unit sales.
If the known amount of coffee in the product differs to this automatic calculation, please enter it over the formula.
This should only be the volume that is used in the customer's own-label products, not across your entire operations.
Finally, select the Certification Type used to certify this volume of coffee-based material used in the product (e.g. Rainforest Alliance - Segregated)
The Summary tab will now provide an overview of the coffee used in the own-label products supplied to each of the customers this manufacturing site supplies.
