Cocoa Declaration Walkthrough



The video above provides an overview of creating and completing your cocoa declaration documents for submission. 

What information is required for the Cocoa survey?

The following information is needed to complete the Cocoa Declaration Form for 2024:


Manufacturing Site Tab

The Manufacturing Site Tab requires the names of the reporting company and its manufacturing site that this declaration form is linked to.

Please note that if you have multiple manufacturing sites, you will have to complete a separate declaration form for each of the manufacturing sites. 

You'll then need to provide the certification status of your manufacturing sites (if applicable) which are producing retailer own-brand label products that contain cocoa. This includes the Certification Number of the certification that is relevant to the manufacturing site and the Expiry Date of the certification.


Product Information Tab

The product information tab is the main part of this declaration form. Here you will need to input information relating to the retailer's own-brand label products that contain cocoa or cocoa-derived material within them.

Product Information

  • Customer's name - Select the customer that this product is sold to.

  • The regional market that the customer sells this product into. Choose this from the drop-down list provided.

  • The commercial category that this product falls under. Select from the drop-down list the category that best represents the product supplied to this customer. 

  • Product name - Enter the product name as used by the customer. 

  • Unique product reference or code (UPC) used by the customer for this product. 


Cocoa Supply Chain Information

  • Type of cocoa product purchased (e.g. cocoa powder or cocoa butter) for use in this product. Select from the drop-down the cocoa type used in this product. If the product uses two different cocoa types, please input two different lines of information.

  • The name of the supplier that supplied the cocoa product to this manufacturing site. 

  • Name of the original cocoa trader or importer. This is the company that purchased the cocoa beans from the country of origin and brought it into the manufacturing market. This could possibly be the same name as the supplier. If you do not know this information please contact your supplier to get this information.

  • The name of the primary processor of cocoa beans into the butter, powder, mass, liquor, or other cocoa ingredient used in this product. Please include the country of processing, in the format Processor Name - Country.

  • The next step is to input the country and sub-national region of origin of the cocoa product used in this product. This is the area where the cocoa was cultivated. If you do not know this information, please write "unknown".


Cocoa Usage & Certification Information

  • Total units of the product (UPC) sold to the customer in 2024

  • Weight per product (UPC) and the unit of reporting (e.g. 200 grams or 0.2 kilograms). If the product is a multipack, please treat the multipack as a single product i.e. use the total weight of the multipack.

  • Cocoa per product and the unit of reporting. Disclose the cocoa per product either in terms of weight (e.g. grams or kilograms) or in terms of percentage. This is the amount of the cocoa-based material (e.g. cocoa butter or cocoa powder) present within each unit of the product.

  • Total tonnage of the cocoa-based material used in this product is automatically calculated based on the Weight per product, Cocoa per product and the 2024 UPC unit sales. 

If the known amount of cocoa in the product differs to this automatic calculation, please enter it over the formula.

  • This should only be the volume that is used in the customer's own-label products, not across your entire operations

  • Finally, select the Certification Type used to certify this volume of cocoa-based material used on the product (e.g. Rainforest Alliance - Segregated) 


The Summary tab will now provide an overview of the cocoa used in the own-label products supplied to each of the customers this manufacturing site supplies. 


Please sense-check this data against your records, save it and upload it to the Mondra platform.
